目的: 比较智能超声眼镜和传统超声在桡动脉置管困难患者中的应用效果。 方法:选择2024年6—11月桡动脉置管困难患者94例,男41例,女53例,年龄≥18岁,BMI 18.5~31.0 kg/m2,ASA Ⅱ或Ⅲ级。采用随机数字表法将患者分为两组:传统超声组(n=44)和超声眼镜组(n=50)。由麻醉科医师实施传统超声或智能超声眼镜引导桡动脉置管,如操作者在10 min内未能成功实施动脉置管,则剔除。记录置管成功时间、置管总次数、置管过程中头部移动次数、首次置管成功例数、动脉导管损坏情况和动脉置管并发症(血肿和痉挛)的发生情况。记录麻醉科医师人体工程学满意情况。 结果:与传统超声组比较,超声眼镜组桡动脉置管成功时间明显缩短(P<0.05),置管总次数、头部移动次数明显减少(P<0.05),动脉导管损坏率及血肿发生率明显降低(P<0.05),首次置管成功率明显升高(P<0.05),麻醉科医师人体工程学满意度明显升高(P<0.05)。 结论:智能超声眼镜可明显缩短动脉置管困难患者置管时间,提高首次置管成功率,提高麻醉科医师人体工程学满意度。 |

Objective: To compare the application effect of smart glasses and traditional ultrasound in patients with difficult radial artery catheterization. Methods: Ninety-four patients with difficult radial artery catheterization from June to November 2024 were enrolled, including 41 males and 53 females, aged ≥ 18 years, BMI 18.5-31.0 kg/m2 and ASA physical status Ⅱ or Ⅲ. The patients were randomly assigned into two groups: the ultrasound group (n = 44) and the smart glasses group (n = 50). Anesthesiologists performed ultrasound or smart glasses-guided radial artery catheterization. If the operator failed to successfully perform arterial catheterization within 10 minutes, the case was excluded. The overall procedure time of arterial cannulation, the first attempt success rate and overall success rate, the overall number of attempts, the overall number of head movements during cannulation, overall complication at the first chosen radial artery, and catheter malfunction were recorded. The anesthesiologists' ergonomic satisfaction was also recorded. Results: Compared with the traditional ultrasound group, the overall procedure time of arterial cannulation in the smart glasses group was significantly reduced (P < 0.05), and the overall number of attempts and the overall number of head movements during cannulation, the damage rate of the arterial duct and the incidence of hematoma were significantly reduced (P < 0.05), the success rate of the first catheterization has significantly increased (P < 0.05), the anesthesiologists' ergonomic satisfaction in the smart glasses group was also significantly improved (P < 0.05). Conclusion: The use of smart glasses in patients with difficult arterial catheterization significantly reduces procedure time and improves the first catheterization success rate and ergonomic satisfaction. |
