麻醉药所致发育期神经毒性(AIDN)是指反复或长期暴露于麻醉药可能会导致的发育期大脑神经损伤。发育期患儿使用麻醉药是否引起神经毒性和神经退行性改变的问题一直备受关注。AIDN的机制涉及全身麻醉药的麻醉效应和非麻醉效应,前者与N-甲基-D-天门冬氨酸(NMDA)和γ-氨基丁酸(GABA)受体有关,后者则源自麻醉药引起的多种生物学效应。本文对儿科手术常用引起AIDN的全身麻醉药、引起发育神经毒性的可能机制以及发育性神经保护策略进行综述,为后续AIDN的研究和患儿神经发育保护提供参考。 |

Anesthetic-induced developmental neurotoxicity (AIDN) refers to the developmental brain nerve damage that may be caused by repeated or long-term exposure to anesthetics. The question of whether the use of anesthetics in developmental children can cause neurotoxicity and neurodegenerative changes has consistently attracted much attention. The AIDN mechanism encompasses both the anesthetic and non-anesthetic effects of general anesthetics. The anesthetic effects are associated with N-methyl-D-aspartic acid (NMDA) and gamma aminobutyric acid (GABA) receptors, whereas the non-anesthetic effects stem from a range of biological impacts induced by the anesthetics. This paper reviews the research progress in the general anesthetics commonly used in pediatric surgery that cause AIDN, the possible mechanism of developmental neurotoxicity, and the current potential protection strategies, which provides a reference for the follow-up study of AIDN and the protection of children's neurodevelopment. |
