预充氧可降低气管插管期间缺氧及死亡率,是快速顺序诱导气管插管的重要步骤。延迟顺序气管插管适用于因快速顺序气管插管而导致严重的低氧血症,且无其他方式进行预充氧的患者,其实质是一种程序性镇静,其主要过程是预充氧。氯胺酮可以保持气道反射和呼吸驱动,是延迟顺序气管插管理想的诱导药。本文就延迟顺序气管插管的概念、预充氧生理机制、影响因素、药物选择、优势以及并发症等方面进行综述,以期为无法正常预充氧和创伤危重患者气道管理提供参考。 |

Preoxygenation reduces hypoxia and mortality during endotracheal intubation and is an important step for rapid sequential induction of intubation. Delayed sequential intubation is suitable for patients whose rapid sequential intubation inevitably causes severe hypoxemia and no other means of preoxygenation, and is essentially a procedural sedation by the process of preoxygenation. Ketamine is an ideal inducer of delayed sequential intubation because it maintains the airway reflex and respiratory drive. This review reviews the concept of delayed sequential intubation, physiological mechanisms, drug selection, implementation steps, advantages and risks of preoxygenation, in order to provide reference for airway management in inadequate preoxygenation and traumatic critically ill patients. |
