单肺通气(OLV)技术可在胸外科手术中更好地控制通气,实现更有效的肺隔离,是胸外科手术中重要的麻醉管理技术。OLV作为机械通气的一种特殊形式,影响正常的肺通气和血流动力学,为麻醉管理带来诸多挑战。心肺之间通过肺循环耦合,两者在解剖和生理功能上存在的相互作用即为心肺交互作用。OLV期间心肺交互作用尤为突出,主要表现为心脏前后负荷、自主神经张力和炎症反应的改变。在OLV手术的围术期,OLV所引起的病理生理变化可能干扰心肺交互作用。本文综述OLV期间心肺之间的三种主要交互作用以及检测方法,为临床优化麻醉管理、改善患者结局提供参考。 |

The one-lung ventilation (OLV) technique can better control ventilation and achieve more effective lung isolation, making it a crucial technique in anesthetic management for such procedures. As a specialized form of mechanical ventilation, OLV presents several challenges in anesthetic care, primarily due to its effects on normal pulmonary ventilation and hemodynamics. The shared thoracic cavity housing the heart and lungs, along with their interconnectedness via the pulmonary circulation, enables a complex interaction between the heart and lungs in terms of anatomical and physiological function, commonly referred to as cardiopulmonary interaction. While such interactions are present in normal physiological states, they are particularly accentuated during OLV. This includes alterations in cardiac preload and afterload, autonomic tone, and inflammatory responses. This review delves into three interactions between the heart and lungs during OLV, and methods for detection. It offers novel insights for optimizing anesthetic management and enhancing patient outcomes. |
