机械通气是全身麻醉的重要组成部分,也是危重患者呼吸支持治疗中不可或缺的手段。但机械通气易诱发炎症反应综合征和多器官功能衰竭,是亟需解决的医学难题。机械通气肺损伤(VILI)涉及复杂的病理过程,包括生物力学应力、氧化应激和炎性介质的释放。本文旨在总结和分析近年来与VILI相关的信号通路,包括炎症通略、氧化应激通路、肺屏障通路和凋亡通路,期望通过分析这些通路的单独或交互作用,为VILI的预防和治疗提供新的思考和见解。 |

Mechanical ventilation, integral to general anesthesia, is a requisite intervention in the respiratory support of critically ill patients. Despite its necessity, it is associated with a propensity to trigger systemic inflammatory response syndrome and subsequent multi-organ dysfunction, constituting an exigent medical conundrum. Ventilator-induced lung injury (VILI) is characterized by intricate pathophysiological mechanisms, encompassing biomechanical stress, oxidative stress, and the liberation of inflammatory mediators. This review endeavors to aggregate and elucidate research on signal transduction pathways implicated in VILI, including inflammatory responses pathways, oxidative stress pathways, lung barrier integrity pathways, and apoptotic processes pathways. The objective is to discern the isolated and synergistic effects of these pathways, thereby contributing novel perspectives and insights into the prophylaxis and therapeutic strategies for VILI. |
