患者在全麻诱导期易出现血流动力学的改变,主要包括气管插管后高血流动力学反应和诱导后低血压两种情况。气管插管后高血流动力学反应可导致患者发生恶性心律失常、心脑血管意外等并发症;诱导后低血压也可引发缺血性脑卒中、心肌缺血等严重的麻醉相关不良事件。因此,维持全麻诱导期的血流动力学稳定非常重要。本文主要从麻醉药物种类的选择、神经阻滞的应用、容量的监测与治疗、诱导药物顺序的优化等方面对全麻诱导期血流动力学管理要点进行综述,为全麻诱导及管理提供参考。 |

Hemodynamic changes can occur frequently during the induction period of general anesthesia, which can be divided into two canditions: high hemodynamic response after endotracheal intubation and post-induction hypotension. High hemodynamic response after endotracheal intubation can lead to complications such as malignant arrhythmia and cardiovascular and cerebrovascular accidents, which can cause serious adverse consequences for patients. Post-induction hypotension is also frequent, and it can cause serious anesthesia-related adverse events such as ischemic stroke and myocardial ischemia. Therefore, it is very important to maintain hemodynamic stability during the induction period of general anesthesia. This review summarizesd the key points of hemodynamic management during the induction period of general anesthesia, including the selection of anesthetic drugs, the application of nerve block, volume monitoring and fluid therapy, and the optimization of induction drug sequence, which provided reference for induction and management of general anesthesia. |
