目的 分析二尖瓣置换或修复术患者舒张期最大跨瓣压与左心房压力(LAP)的相关性。 方法 选择行心肺转流(CPB)下二尖瓣置换或修复手术的瓣膜病患者20例,男9例,女11例,年龄18~80岁,术中行经食管超声心动图(TEE)监测。于心脏复跳后CPB停机前、左心房引流管拔除前,分别采用左心房引流管测量LAP,TEE测量人工二尖瓣舒张期跨瓣压,中心静脉导管测量中心静脉压(CVP)。采用Pearson相关性分析评价人工二尖瓣舒张期最大跨瓣压与LAP的相关性。 结果 患者的LAP为(13.9±2.6)mmHg,人工二尖瓣舒张期最大跨瓣压为(12.3±2.3)mmHg,CVP为(8.6±2.1)mmHg。人工二尖瓣舒张期最大跨瓣压与LAP高度相关(r=0.86,P<0.001),CVP与LAP中度相关(r=0.60,P<0.001)。 结论 人工二尖瓣舒张期最大跨瓣压与左心房压力高度相关,可反映左心房压力的变化及趋势。 |

Objective To analyze the correlation between the diastolic peak pressure gradient of the mitral valve and the left atrial pressure (LAP) in patients undergoing mitral valve replacement or repair. Methods Twenty patients with valvular disease who needed cardiac mitral valve repair or replacement under cardiopulmonary bypass (CPB), 9 males and 11 females, aged 18-80 years, were enrolled and observed. Transesophageal echocardiography (TEE) monitoring was used during the operation. Before the CPB stopped (after the heart rebeat), and before the left atrial drainage tube was removed, the left atrial drainage tube was used to measure the LAP, TEE was used to measure the diastolic peak pressure gradient of the mitral valve. The central venous pressure (CVP) was measured through a central venous catheter. Pearson's correlation analysis was used to evaluate the correlation between the diastolic peak pressure gradient of the artificial mitral valve and LAP. Results The measured LAP was (13.9 ± 2.6) mmHg, the diastolic peak pressure gradient of the mitral valve was (12.3 ± 2.3) mmHg, and the CVP was (8.6 ± 2.1) mmHg. LAPs and peak pressure gradients were highly correlated (r = 0.86, P < 0.001), while LAPs and CVPs were moderately correlated (r = 0.60, P < 0.001). Conclusion The diastolic peak pressure gradient of the artificial mitral valve is highly correlated with LAP, and better reflects the changes and trends of left atrial pressure. |
