目的 探讨术前父母焦虑对患儿全麻术后行为改变的影响。
方法 选择择期行耳鼻喉科手术患儿60例,男41例,女19例,年龄2~8岁,ASA Ⅰ或Ⅱ级,手术时间<60 min,麻醉方式为全身麻醉。术前及手术日评估父母焦虑评分,将患儿父母分为焦虑组(A组)和无焦虑组(B组),患儿进入其父母相应的组内。记录患儿的等待区和诱导时焦虑评分、手术时间、苏醒时间、拔管后苏醒期躁动评分和苏醒期躁动发生率。分别在术前1 d、术后3 d、1周、2周和1个月采用术后行为量表进行术前问卷调查和术后电话随访,记录患儿术后行为改变的发生情况。
结果 A组患儿的等待区焦虑评分和苏醒期躁动发生率明显高于B组(P<0.01)。父母焦虑评分与患儿等待区焦虑评分和苏醒期躁动评分都呈中等程度的相关性(r=0.487,P<0.01和r=0.574,P<0.01)。A组的患儿术后3 d、1周和2周行为改变发生率明显高于B组(P<0.05)。
结论 术前父母焦虑可明显增加行耳鼻喉科手术患儿的术后行为改变。

Objective To explore the effect of parental anxiety on postoperative behavioral outcomes of children having general anesthesia.
Methods Sixty children who would receive ear-nose-throat (ENT) surgery, 41 males and 19 females. The children were aged 2-8 years, falling into ASA physical status Ⅰ or Ⅱ category, completed operative time less than 60 min, under general anesthesia. Parental anxiety scores were assessed before surgery and operation day,and then the children were divided into parental anxiety group (group A) and non-anxiety group (group B). The anxiety score of the child in the waiting area and during the induction were recorded, as well as the operation time, recovery time and the emergence agitation scores after extubation. Posthospitalization behaviour questionaire and postoperative telephone follow-up were performed on 1 day before surgery, 3 days, 1 week, 2 weeks, and 1 month after surgery to observe the changes of postoperative behavior.
Results The anxiety scores in the waiting area and the emergence agitation scores were significantly higher in group A than those in group B (P < 0.01). Parental anxiety scores were moderately correlated with anxiety scores in the waiting area and the emergence agitation scores during recovery period(r = 0.487, P<0.01 and r = 0.574, P<0.01). The incidence of adverse behaviors in group A was significantly higher than that in group B at 3 days, 1 and 2 weeks after surgery (P<0.05).
Conclusion Preoperative parental anxiety can significantly increase the incidence of postoperative behavioral outcomes in children with ENT surgery.
