目的 观察经皮电刺激内关穴和曲池穴对全麻气管插管患者应激反应的影响。
方法 选取择期行腰椎手术患者90例,男36例,女54例,年龄40~65岁,ASA Ⅰ或Ⅱ级,将患者随机分为三组: 经皮穴位电刺激组(A组)、经皮非穴位电刺激组(NA组)和非电刺激组(NS组),每组30例。三组患者全麻方案相同,A组术前经皮电刺激内关穴和曲池穴30 min;NA组术前在非穴位经皮电刺激30 min;NS组不做任何处理,实施标准全麻方案。记录插管前、插管时和插管后3 min SBP、DBP和HR。同时于以上时点经桡动脉采血,测定血浆肾上腺素(E)和去甲肾上腺素(NE)浓度。
结果 插管前三组SBP、DBP、HR及血浆E和NE浓度差异无统计学意义。与插管前比较,插管时和插管后3 min三组SBP、DBP及血浆E和NE浓度明显升高,HR明显增快(P<0.05)。与A组比较,插管时NA组和NS组SBP、DBP及血浆E和NE浓度明显升高,HR明显增快(P<0.05)。
结论 经皮电刺激内关穴和曲池穴对全麻气管插管引起的心血管反应表现出很好的减轻作用。

Objective To observe the effect of transcutaneous electrical stimulation at Neiguan and Quchi acupoints on stress response to tracheal intubation under general anesthesia.
Methods Ninety patients scheduled for lumbar surgery, 36 males and 54 females, aged 40 - 65 years, falling into ASA physical status Ⅰ or Ⅱ, were selected. The patients were randomly divided into three groups (n=30): transcutaneous electrical acupoint stimulation group (group A), transcutaneous electric non-acupoint stimulation group (group NA) and non-stimulation group (group NS). All patients in three groups had followed a same anesthetic protocol, except for the intervention of patients in group NS without any treatment, the patients in group A received transcutaneous electrical acupiont stimulation at Neiguan, Quchi acupoints for 30 min and patients in group NA stimulated at the non-acupoint by transcutaneous electrical stimulation for 30 min. The systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) were recorded at intubation, before and 3 min after intubation. Meanwhile, the blood sample, for plasma epinephrine and norepinephrine, was taken from the radial artery.
Results There was no significant difference in SBP, DBP, HR, E and NE among the three groups before intubation. Compared with pre-intubation, SBP, DBP, E and NE in plasma and HR were significantly increased at intubation and 3 min after intubation among the three groups (P < 0.05). Compared with group A, SBP, DBP, E and NE in plasma and HR in group NA and group NS increased significantly at intubation (P < 0.05).
Conclusion Percutaneous acupoint electrical stimulation of Neiguan and Quchi has a good preventive effect on cardiovascular response induced by tracheal intubation under general anesthesia.
