目的 观察不同程度高碳酸血症预处理对大鼠肺缺血-再灌注损伤(lung ischemia reperfusion injury, LIRI)的影响。

方法 雄性SD大鼠50只, 2~3月龄, 随机分为五组, 每组10只。假手术组(S组): 开胸后游离左侧肺门, 不予其他处理;IR组: 采用左肺原位缺血45 min, 再灌注180 min建立大鼠左肺IR模型;L、M、H组: 通过调节RR,使PETCO2分别达到46~55 mmHg(L组)、56~65 mmHg(M组)、66~75 mmHg(H组), 预处理5 min, 然后同IR组处理建立大鼠左肺IR模型。缺血-再灌注(ischemia-reperfusion, IR)180 min后, 采用ELISA法检测血清IL-8和IL-10浓度;处死大鼠, 取肺组织标本, 采用考马斯亮蓝染色法检测肺泡灌洗液(bronchial alveolar lavage fluid, BALF)中总蛋白(TP)含量, 肺组织湿/干重比(W/D)、丙二醛(MDA)含量、超氧化物歧化酶(SOD)活性, 观察肺组织病理学变化及TNF-α蛋白阳性细胞面积。

结果 与S组比较, IR、L、M、H组血清IL-8浓度、肺组织病理学评分、肺组织W/D、MDA含量, 以及TNF-α蛋白阳性细胞面积百分比明显升高(P<0.05), SOD活性明显降低(P<0.05)。与IR组比较, L、M、H组血清IL-8浓度、肺组织病理学评分、肺组织W/D、MDA含量, 以及TNF-α蛋白阳性细胞面积百分比、SOD活性明显降低(P<0.05)。与L组比较, H组以上各项指标均明显降低(P<0.05)。IR组血清IL-10浓度明显高于S组(P<0.05)。

结论 通过改变呼吸模式造成高碳酸血症预处理, 可以抑制大鼠LIRI后氧自由基爆发和促炎细胞因子生成, 降低早期炎症反应水平, 减少炎性渗出, 对减轻LIRI起到一定的作用, 而且在PETCO2 46~75 mmHg内, 随着PETCO2升高,作用越强。

Objective To observe the effects of hypercapnia pretreatment on lung ischemia-reperfusion (IR) injury.

Methods Fifty male Sprague Dawley rats, aged 2-3 months, were randomly divided into five groups. Control group (group S) was continuously perfused without ischemia after free left hilar; group IR was performed occlusion of the left pulmonary hilum for 45 min, followed by reperfusion 180 min; preconditioning group was adjusted respiratory rate to mantain PETCO2 at 46-55 mmHg (group L), 56-65 mmHg (group M) and 66-75 mmHg (group H), respectively, and then treated in a way same as group IR was. After 180 min of IR, serum IL-8 and IL-10 concentrations were measured by ELISA; rats were sacrificed and lung tissue samples were taken to detect total protein (TP) content in bronchoalveolar lavage fluid (BALF) by coomassie blue staining; wet/dry weight ratio (W/D), malondialdehyde (MDA), the concentrations of IL-8 and IL-10, and the activity of superoxide dismutase (SOD) were measured. The pathological changes of lung tissue and the expression of TNF-α immunoreactive substance were observed.

Results Compared with group S, the concentration of IL-8 in serum, lung histopathological score, W/D, MDA concentration, and TNF-α protein positive cell area percentage were significantly increased in groups IR, L, M and H (P < 0.05), SOD activity was significantly reduced (P < 0.05). Compared with group IR, the all indexes of groups L, M and H decreased (P < 0.05). Among the groups L, M and H, the indicators of group H were lower than group L (P < 0.05). The concentration of IL-10 in group IR was significantly higher compared with group S (P < 0.05).

Conclusion Hypercapnia pretreatment can be achieved by changing the breathing pattern. It can inhibit the generation of oxygen free radicals and proinflammatory cytokines, and reduce the level of early inflammatory reaction and the inflammatory exudation on LIRI in rats. The protective effect increases along with PETCO2 increasing.
