目的 通过序贯法测定羟考酮用于显微血管减压手术术终镇痛衔接的半数有效剂量(Median effective dose, ED50)。

方法 选择2017年6—10月在全麻下行经乙状窦入路显微血管减压手术的面肌痉挛患者23例, 男5例, 女18例, 年龄18~64岁, BMI 18~24 kg/m2, ASA Ⅰ或Ⅱ级。手术结束前30 min给予羟考酮, 手术结束后连接相同配方的镇痛泵。根据序贯法设定浓度, 当出现第7个交叉点时终止试验。术后30 min患者镇痛口述分级(verbal rating score, VRS)评分1分为阴性。

结果 Dixon-Mood法计算羟考酮ED50为0.054 mg/kg(95%CI 0.053~0.056 mg/kg);Probit回归模型计算为0.054 mg/kg(95%CI 0.030~0.077 mg/kg)。

结论 羟考酮用于显微血管减压手术术终镇痛衔接的ED50为0.054 mg/kg。

Objective To determine median effective dose (ED50) of oxycodone hydrochloride for postoperative analgesia after microsurgical vascular decompression by sequential method.

Methods Twenty-three patients underwent transvascular sigmoid sinus microsurgical vascular decompression under general anesthesia, 5 males and 18 females, aged 18-64 years, BMI 18-24 kg/m2, ASA physical status Ⅰ or Ⅱ, were given oxycodone hydrochloride 30 minutes before the end of surgery, and the analgesia pump of the same formula was given after the operation. The concentration was set according to the principle of sequential method, and the experiment was terminated when the 7th crossing point occurring. 30 min after operation, the grading of analgesia, orally scored 1 by patients, was considered as negative.

Results The ED50 of oxycodone hydrochloride for postoperative analgesia of microsurgical vascular decompression was calculated by Dixon-Mood as 0.054 mg/kg (95% CI 0.053-0.056 mg/kg). The Probit regression model calculated it as 0.054 mg/kg (95% CI 0.030-0.077 mg/kg).

Conclusion The ED50 of oxycodone hydrochloride for postoperative analgesia after microsurgical vascular decompression is 0.054 mg/kg.
