目的 观察术前行星状神经节阻滞(stellate ganglion block,SGB)对大鼠腹部胃肠手术及非胃肠手术后胃肠功能的影响。方法 健康雄性SD大鼠54只,体重(200±10)g,随机分为:正常对照组(C组,n=6)、腹部胃肠手术组(G组,n=12)、SGB+腹部胃肠手术组(SG组,n=12)、腹部非胃肠手术组(NG组,n=12)、SGB+腹部非胃肠手术组(SNG组,n=12)。C组不给予任何操作。分别于SGB前(T1)、术毕(T2)、术后12 h(T3)及24 h(T4)经大鼠锁骨下静脉采集血样检测血清去甲肾上腺素(NE)、肿瘤坏死因子α(TNF-α)、白细胞介素 6(IL-6)浓度; T3和T4时每组随机取6只大鼠测小肠推进率,处死后取胃窦部、结肠组织,采用免疫组织化学法测定大鼠胃窦部和结肠近端胃动素(MTL)、血管活性肠肽(VIP)的表达。结果 与T1和C组比较,T2~T4时G组、SG组、NG组、SNG组血清NE、TNF-α、IL-6浓度均明显升高(P<0.05),且G组明显高于SG组(P<0.05),NG组明显高于SNG组(P<0.05)。与T3和C组比较,T4时G组、SG组、NG组、SNG组小肠推进率、MTL明显升高(P<0.05),且SG组明显高于G组(P<0.05),SNG组明显高于NG组(P<0.05);VIP表达明显降低(P<0.05),且明显高于C组(P<0.05),SG组明显低于G组(P<0.05),SNG组明显低于NG组(P<0.05)。结论 术前星状神经节阻滞能够促进大鼠腹部手术后胃肠功能的恢复,可能是通过降低应激反应、炎性反应,调节胃肠激素等机制实现。 |

Objective To observe the effect of preoperative stellate ganglion block (SGB) on gastrointestinal function in rats undergoing gastrointestinal and non gastrointestinal abdominal surgery. Methods Fifty four male SD rats, weighing (200±10)g, were randomly divided into 5 groups: control group (n=6, group C), gastrointesinal abdominal surgery group (n=12, group G), SGB+gastrointesinal abdominal surgery group (n=12, group SG), non gastrointestinal abdominal surgery group (n=12, group NG), SGB+non gastrointestinal abdominal surgery group (n=12, group SNG). Group C was given no treatment. Venous blood samples were taken for determination of the levels of serum NE, TNF-α and IL-6 at the time before stellate ganglion block (T1), the end of operation (T2), 12 h (T3) and 24 h after operation (T4). Six rats were randomly chosen from each group to test bowel propulsion rate at T3 and T4, then sacrificed to measure motilin (MTL) and vasoactive intestinal peptide (VIP) in antrum and proximal colon. Results Compared with T1 and group C, the serum concentrations of NE,TNF-α and IL-6 significantly increased at T2-T4 in groups G, SG, NG and SNG (P<0.05). The NE,TNF-α and IL-6 levels in group G were significantly higher than those in group SG (P<0.05), and the serum NE,TNF-α and IL-6 levels in group NG were significantly higher than those in group SNG (P<0.05). Compared with T3 and group C, the bowel propulsion rate and MTL levels significantly increased at T4 in groups G, SG, NG and SNG (P<0.05).The bowel propulsion rate and MTL levels in group SG and group SNG were significantly higher than those in group G and group NG, respectively (P<0.05). The VIP levels significantly decreased in groups G, SG, NG and SNG than that in group C (P<0.05). The VIP levels in group SG were significantly lower than those in group G (P<0.05), and the VIP levels in group SNG were significantly lower than those in group NG (P<0.05). Conclusion Preoperative stellate ganglion block can premote the recovery of gastrointestinal function in rats after abdominal surgery, attenuated stress response and inflammatory reaction, and regulation of gastrointestinal hormone level may be involved in the mechanism. |
