Postoperative sleep disturbance (PSD) is a common surgical complication influenced by pain, inflammation, anesthetics, comorbidities, and environmental factors. PSD induces neuroendocrine imbalance, impairing wound healing and immune function while increasing risks of delirium and chronic pain. In recent years, beyond optimizing analgesic protocols, the perioperative prophylactic use of drugs with sleep-regulating properties like dexmedetomidine, as well as non-pharmacological interventions such as transcutaneous electrical acupoint stimulation, has shown promising application potential. Systematic risk identification and targeted prevention are thus vital for improving outcomes. This review summarizes recent advances in the mechanisms and therapeutic interventions for PSD, aiming to provide evidence and reference for optimizing perioperative sleep management.
