| 气管插管是麻醉和重症监护中广泛使用的重要操作,但气管插管过程中的机械刺激和气道反射常引发一系列应激反应,包括显著的心血管波动、炎症反应及氧化应激,可能导致术中及术后并发症,严重者甚至危及生命。近年来,围绕气管插管应激反应的发生机制及其药物干预策略研究取得了显著进展。应激反应主要机制涉及交感神经系统的过度激活、炎性因子的释放及氧化应激的加剧,这些机制相互作用构成复杂的生理调节网络。针对不同的生理环节,药物干预已展现出显著的临床价值。因此,本文综述了气管插管应激反应的发生机制,并重点探讨了镇静药、β受体阻滞剂及抗氧化剂等药物在减轻应激反应中的应用及研究进展,旨为未来临床优化干预策略、多靶点联合干预策略的开发、个性化用药方案的优化以及基于分子机制的新型药物研发提供参考。 |

| Tracheal intubation is a widely used critical procedure in anesthesia and intensive care, but the mechanical stimulation and airway reflexes during the process often trigger a series of stress responses, including significant cardiovascular fluctuations, inflammatory responses, and oxidative stress, which may lead to intraoperative and postoperative complications and even threaten life. In recent years, significant progress has been made in the study of the mechanisms of stress responses to tracheal intubation and drug intervention strategies. The main mechanisms of stress responses involve excessive activation of the sympathetic nervous system, release of inflammatory factors, and aggravation of oxidative stress, which interact with each other to form a complex physiological regulation network. Drug intervention has shown significant clinical value at different physiological stages. Therefore, this review summarizes the mechanisms of stress responses to tracheal intubation and focuses on the application and research progress of sedatives, beta-receptor blockers, and antioxidants in reducing stress responses, with the aim of providing reference for future clinical optimization of intervention strategies, the development of multi-targeted combined intervention strategies, the optimization of personalized drug regimens, and the development of novel drugs based on molecular mechanisms. |
