目的：系统评价穴位贴敷防治骨科术后恶心呕吐的效果。
方法：计算机检索PubMed、Cochrane Library、中国生物医学文献服务数据库、Web of Science、Embase、中国知网等数据库中关于穴位贴敷防治骨科手术后恶心呕吐的随机对照研究，检索时间为建库至2025年3月20日，采用RevMan 5.4进行Meta分析。
结果：最终纳入10篇文献，包括1 319例患者，其中中药穴位贴敷组661例，对照组658例。Meta分析结果显示，与对照组比较，中药穴位贴敷组骨科手术后恶心呕吐发生率明显降低（RR＝0.56，95%CI 0.48～0.65，P＜0.001），术后恶心呕吐程度VAS评分明显降低（MD＝-0.95分，95%CI -1.47～-0.42分，P＜0. 001）。亚组分析显示，与对照组比较，脊髓麻醉或腰-硬联合麻醉下骨科手术中药穴位贴敷组术后恶心呕吐发生率明显降低（RR=0.58，95%CI 0.49～0.69，P＜0.001），全麻下骨科手术两组术后晚期恶心呕吐发生率差异无统计学意义。纳入研究中药穴位贴敷期间均未出现局部皮肤过敏等不良反应。
结论：中药穴位贴敷可降低骨科术后恶心呕吐发生率及严重程度，且无明显不良反应发生。 |

Objective: To systematically evaluate the effect of acupoint herbal plaster for prevention and treatment of postoperative nausea and vomiting after orthopedic surgery.
Methods: Randomized controlled trials about acupoint herbal plaster for prevention and treatment of postoperative nausea and vomiting in orthopedic surgery were systematically searched from databases, including PubMed, Cochrane Library, SinoMed, Web of Science, Embase, CNKI and so on. The retrieval period was from establishment of databases to March 20, 2025. RevMan 5.4 software was used for meta-analysis.
Results: Ten literatures involving 1 319 patients were included, including 661 in the acupoint herbal plaster group and 658 in the control group. The results of meta-analysis showed that, compared with the control group, acupoint herbal plaster group can reduce the incidence of postoperative nausea and vomiting after orthopedic surgery (RR = 0.56, 95%CI 0.48 to 0.65, P < 0.001) and reduce the VAS score of postoperative nausea and vomiting degree (MD = -0.95,95% CI -1.47 to -0.42, P < 0.001). Subgroup analysis showed that, compared with the control group, acupoint herbal plaster group can significantly reduce the incidence of postoperative nausea and vomiting under spinal anesthesia or combined spinal-epidural anesthesia in orthopedic surgery (RR = 0.58, 95% CI 0.49 to 0.69, P < 0.001). However, there were no statistically significant differences in the incidence of late postoperative nausea and vomiting between two groups under general anesthesia for orthopedic surgery. No adverse reactions such as local skin allergy occurred during the period of traditional Chinese medicine acupoint application in the studies.
Conclusion: Acupoint application of traditional Chinese medicine can reduce the incidence and severity of postoperative nausea and vomiting in orthopedic surgery, and there are no obvious adverse reactions. |
