目的:基于指南研究与评价工具(AGREEⅡ)对2023年麻醉相关临床实践指南进行方法学质量评价,了解麻醉相关指南的证据基础、筛选高质量指南、提高医学决策水平。
方法:系统检索中国知网、中国生物医学文献服务系统(CBM)、万方、Web of Science、PubMed、Embase及美国国立指南数据库(NGC)、英国国家卫生与临床优化研究所(NICE)、国际指南数据库(GIN)、苏格兰校际指南网(SIGN)、中国医脉通指南网等指南相关网站,纳入麻醉相关临床实践指南,检索时间为2023年1—12月。采用AGREE Ⅱ指南评价工具对纳入的指南进行方法学质量评价和分析。
结果:共纳入26篇指南,其中22篇英文指南,4篇中文指南。纳入指南的总体质量评价结果如下:A级2篇、B级22篇、C级2篇。AGREE Ⅱ中6个领域平均标准化得分分别为:范围和目的(80%)、参与人员(55%)、严谨性(48%)、表达的明晰性(75%)、应用性(49%)、编辑独立性(57%)。中文和英文指南的平均标准化得分分别为:范围和目的(75% vs. 81%)、参与人员(46% vs. 57%)、严谨性(30% vs. 52%)、表达的明晰性(61% vs. 77%)、应用性(29% vs. 53%)、编辑独立性(34% vs. 61%)。
结论:2023年发布的麻醉相关指南在方法学质量上整体有待提升,其在编辑独立性、严谨性、应用性方面得分相对较低,且中英文指南质量水平存在差异。未来的临床实践指南应积极借鉴国际先进经验与技术成果,推动麻醉学技术的普及与进步。

Objective: To evaluate the methodological quality of anesthesia-related clinical practice guidelines published in 2023 based on the AGREE Ⅱ guideline evaluation tool, gain a better understanding of their evidence base, and screen high-quality guidelines to improve medical decision-making.
Methods: Systematically searched guideline-related websites, including CNKI, China Biomedical Literature Service System (CBM), Wanfang, Web of Science, PubMed, Embase, the National Guideline Clearinghouse (NGC) of the United States, the National Institute for Health and Care Excellence (NICE) of the United Kingdom, the Guidelines International Network (GIN), the Scottish Intercollegiate Guidelines Network (SIGN), and the Chinese YimaiTong Guideline Network. The search period was from January to December, 2023. The AGREE Ⅱ guideline evaluation tool was used to evaluate and analyze the methodological quality of the included guidelines.
Results: A total of 26 guidelines were included, comprising 22 English-language and 4 Chinese-language guidelines. Overall quality evaluation showed that, 2 guidelines rated A, 22 rated B, and 2 rated C. The average standardized scores across the six AGREE Ⅱ domains were: scope and purpose (80%), stakeholder involvement (55%), rigor of development (48%), clarity of presentation (75%), applicability (49%), and editorial independence (57%). Comparative analysis between Chinese and English guidelines revealed average standardized scores as follows: scope and purpose (75% vs. 81%), stakeholder involvement (46% vs. 57%), rigor of development (30% vs. 52%), clarity of presentation (61% vs. 77%), applicability (29% vs. 53%), and editorial independence (34% vs. 61%).
Conclusion: The anesthesiology-related guidelines published in 2023 generally need improvement in methodological quality. The guidelines scored relatively low in editorial independence, rigor, and applicability, and there were differences in the quality levels between Chinese-language and English-language guidelines. Future clinical practice guidelines should actively draw on international advanced experiences and technological achievements to promote the popularization and progress of anesthesiology technologies.
